Fees for Service
The following fees below will apply to your invoice depending on how records will be requested to be received.
- Medical records fee will be at $48.00 + $0.10 per page which includes the retrieval/clerical/scan/storage fee.
- Billing records fee will be at $24.00 + $0.10 per page which includes the retrieval/clerical/scan/storage fee.
- Films Request fee is $24.00 for X-ray/MRI Breakdown. The duplication fees are $25.00 to duplicate X-ray Films and $35.00 for MRI Films/CT Scans. X-ray and MRI films will be provided on a CD/DVD only.
- $15 fee will be charged if no records are found. A declaration of no records will be provided once payment is received.
- All transaction will be charged an Electronic Processing Fee of $2.99.
- If payment is made by check there will be a $10 Check Fee

Cancellation Fee
- All cancelled orders will be charged $15 plus Electronic Processing Fee of $2.99 before records order has been fulfilled.
